“We have been working with AzHAC for just over a year. They have 5 years of HMIS data that they have been analyzing in a number of ways. However, one of the specific things that we have looked into is those who are high utilizers of the HMIS system. Through their analysis, we have identified the top 5% of users, what resources they are using, and how we can better serve those individuals. As we are redesigning our Coordinated Entry system, we are leaning more into data out of HMIS rather than a series of questions to the individual or family. One of the categories that is part of the new assessment is High Utilizers based on the analysis done by AzHAC. It has been very helpful to understand the movement of individuals and families through the system and has changed the way we are planning to prioritize in the future.”
